PUBG Mobile Form Request Feature Still Locked on My Account

I have been logging into my PUBG Mobile account regularly for at least 15 consecutive days, yet I am still unable to submit a form request. My in-game ID is 5890***45. Even though I have fulfilled the required waiting period, the form request screen has not become available, preventing me from completing the necessary process.

This issue has persisted since the 15th day, and I no longer believe it will resolve itself automatically. If there is any restriction, suspension, or technical issue affecting my account, I request that it be clearly explained to me. If no such issue exists, I ask that my access to the form request feature be activated as soon as possible.
