Unauthorized Mental Impulse Charges, Request Cancel Subscription

I have been charged 39.98 USD every month by “IQ Mental Impulse” for at least the last six months, even though I do not know what this service is and do not recall ever subscribing to it. In July I was even charged twice. I only realized the extent of the issue after reviewing my Huntington bank statements. The most recent charges I have identified on my Huntington account are 39.98 USD on 26.03.2026, 39.98 USD on 23.04.2026, 39.98 USD on 21.05.2026, 39.98 USD on 18.06.2026 and 39.98 USD on 16.07.2026, all under the name IQ Mental Impulse. From the last few months alone I estimate the total taken to be around 200 USD, and there may be earlier charges as well. I request that you immediately cancel and terminate any subscription, account, or recurring billing agreement in my name related to IQ Mental Impulse so that no further payments are taken. I also request a full refund for at least the last four months of charges and the July double charge, and a clear explanation of how this subscription was created and authorized without my informed consent.
