PhonePe Wallet Top-Up Failed - Amount Debited But Not Credited

I am writing regarding a failed transaction on PhonePe while adding money to my wallet. On 05 December 2025, I attempted to add ₹3,000 to my PhonePe wallet from my Union Bank of India account using UPI. The amount was debited from my bank account, but it has not been credited to my PhonePe wallet or returned to my bank account. This transaction was made using my UPI ID 85188 88598@IOB. Since then, the money has remained missing, and my PhonePe wallet balance has not changed accordingly. I request that this issue be investigated urgently and that the missing ₹3,000 be either credited to my PhonePe wallet or refunded back to my Union Bank of India account as soon as possible. I also request a clear explanation of why this failure occurred so that similar issues do not happen again.
