Pfizer Ngenla 60 mg Pen Defect Caused Over-Dispensing of Pediatric Growth Hormone

We use Pfizer's Ngenla 60 mg hormone injection solution for our child's medical treatment. While we experienced no issues with the previous four pens we used, we encountered a severe dosing problem with the fifth pen.
The pen in question is an Ngenla 60 mg pen, Lot number ME4779, with an expiration date of 01/2028. On May 3, 2026, and May 10, 2026, we set the dose to 20 mg and administered it exactly as instructed by our doctor. However, after both applications, we noticed that approximately 27 mg had been depleted from the pen's contents instead of 20 mg. As a result, only about 6 mg remained in the 60 mg pen, making it impossible to administer the planned third dose.
Initially, we thought it might be user error, so we performed the second application very carefully following the exact same instructions, but the exact same over-dispensation occurred again. The fact that no similar issues occurred with the previous four pens suggests that the problem may be a technical flaw unique to this specific pen.
Our treatment continues under medical supervision, and we perform all dosing strictly as directed by our physician. We reported the issue to the pharmaceutical representative and our dispensing pharmacy, but we have received no solution or follow-up so far. This situation has created a risk of disruption in our child's ongoing treatment.
We demand that this dosing problem be investigated urgently, that technical support be provided regarding this product, and that a replacement pen be supplied for this defective unit so that our treatment is not interrupted and our hardship is resolved.
