SLY Number $14.99 Unauthorized Charges - Refund Request

On 2/2/2026 I discovered two charges of 14.99 each made by SLY Number on my card ending in 4200. I have never opened an account with SLY Number, never authorized these payments, and this situation is clearly related to identity theft. I attempted to contact SLY Number by email but have not received any response, and I could not find a working customer service phone number, which has left me without any support channel to resolve this issue. I request that both of these 14.99 charges be fully refunded, that any account or service opened in my name be identified and closed, and that an identity theft investigation be carried out. I also ask for a written confirmation explaining the actions taken and the results of the investigation so that I can use it in any further steps with my bank and the relevant authorities.
