Facebook Account Hacked: Recover ID & Reconnect Phone and Gmail

My Facebook account was hacked about two to three months ago and since then I have lost access to it. Someone else is controlling my Facebook ID and I cannot log in or recover it. I want my hacked Facebook ID to be recovered and access to be given back only to me. I request that my own phone number and Gmail be reconnected to this account so that no one else can use my profile and information without my permission.
